DVA Health Cards (White and Gold) are accepted as payment upon a GP referral

As DVA registered exercise physiologists, our team is able to provide exercise physiology to White and Gold DVA card holders. DVA cards allow you to receive exercise physiology to help manage and rehabilitate persistent pain, physical injury, mental health conditions and neurological disorders.

Our DVA exercise physiology program is no different to any of our other exercise physiology services. Our team work 1:1 with you to listen to your story, learn your goals and develop an exercise and wellness program that meets your current capacity and needs.

How can exercise physiology help?

Our studio combines commercial-grade exercise equipment with a private space to ensure we have the right environment for you to excel. With easy parking available and a bathroom on-site, our studio is accessible to people from all walks of life.

What is the difference between a white and gold card?

It is important that you obtain a D904 GP referral (found here) before your first appointment, regardless of what colour card you hold.

White card

Your exercise physiology treatment is targeted towards rehabilitating/managing a specific, accepted condition as specified by your GP. After a treatment cycle (up to 12 sessions), we will report back to your GP and if required, another referral will be needed for the next treatment cycle.

Gold card

Your exercise physiology treatment is more versatile, meaning we can help you with rehabilitating/managing any of the conditions that you may be living with. After a treatment cycle (up to 12 sessions), we will report back to your GP and if required, another referral will be needed for the next treatment cycle.

Like a regular gold card, we have versatility with what conditions exercise physiology can treat. We can work with you to manage any condition you may be living with. However, no referral is required after a treatment cycle.

 

 

Meet Mark

Mark is a DVA white card holder with lumbar spondylosis as his accepted condition. Mark lives with persistent lower back pain that impairs his ability to care for his granddaughter.

Mark visited his GP to obtain a D904 referral before booking an appointment at AXM Exercise Physiology.

Mark attended his initial assessment and discussed with his exercise physiologist the impact of his condition and his treatment goals. Mark then underwent an assessment to help develop a treatment plan for his injury.

Mark has begun attending 1:1 sessions with his exercise physiologist. His program is tailored to build strength and confidence in his lower back. If after 12 sessions, Mark requires further treatment to achieve his goals, he can visit his GP to gain another referral to continue his exercise physiology journey.
